We understand that every office is a little different and we hope that the information provided below will help you become more equated with ours.
Prescription Refills
To ensure we are providing you with the prescriptions that you need, please bring your bottles or list of medication to every office visit as we can renew or refill prescriptions during this time. If you need a prescription refill, please have your pharmacy send the request to our office. This works best if you call your pharmacy directly, even if you have no refills left.
Please plan ahead as most local pharmacies request 2-3 business days for prescription refills. For mail order prescriptions, please be sure to contact your pharmacy at least 2 weeks before you will need the refill via their 800 number or website. We realize emergencies happen, but ask that you call our office and not come to the office requesting an emergency refill. This interrupts the care we give patient who are scheduled to be seen.
Patient Registration
Our patient registration form must be completed annually. We understand that repeating this process every year is inconvenient but insurance and HIPPA regulations require that we confirm and update all this information annually. All of our forms are available in our Resource information page.
Appointment Day, what do I need to bring?
On the day of your appointment, please make sure you bring:
* Updated insurance card
* Prescriptions bottles or list of current medications
* Completed New Patient paperwork, yearly registration paperwork, or forms needed for appointment. Emergencies can sometimes arise and may disrupt our schedule, so we appreciate your patience and understanding when this occurs.
Canceled or “No Show” Appointments
Please allow 24 hours for cancellation of an appointment as a courtesy to our office. If for some reason you neglect to notify us and ‘’No Show’’ for an appointment, you will receive a letter the first time. After second ‘’No Show” occurs there will be a $25 charge to your account. Multiple “No Shows” may result in being dismissed from the practice.
Insurance and Payments
We will bill your insurance as a courtesy. Co-payments and deductibles are to be paid at time of your visit. We will estimate the balances to the best of our ability. However, you still may end up owing a small amount once the claim is processed. Please understand that insurance reimbursement can be a long and difficult process. In fact, insurers will routinely stall, deny, and reduce payments. To that end, our billing staff has undergone extensive training to maximize your insurance reimbursement, while reducing your time by which they pay.
Can someone else bring my child? Children or Adolescence being seen under the age of 18 without a parent
Orange Family Physicians must receive permission from a child’s legal parent or guardian before providing treatment for a non-emergent injury or illness. If the consent is not on file with us or presented at the time of service Orange Family will contact the child’s parent or legal guardian before he or she is seen by the physician. Consent to Release Information
We are committed to protect the privacy of your personal information, in order to release any medical or financial information to someone other than the patient a consent must be completed. This consent will remain in place until otherwise revoked or changed by the patient.
After Hours
If a situation arises after hours our answering service will speak with you and can page the physician on call. You can reach our after hours service by calling 877-523-5826.
Holiday Hours
We are closed on major holidays including New Year’s Day, Memorial Day, Independence Day, labor Day, Thanksgiving Day and Christmas Day.
Sports Physicals
The Virginia High School League requires the completion of their physical form prior to athletic participation. You must have a physical exam after May 1 of the current school year to meet the criteria. Our office is happy to complete these physicals.
Is there a lab on site?
LabCorp is located on site Monday through Friday from 8 a.m. to 5 p.m. If labs need to be completed at another facility due to insurance or billing issues please let our office know and we will be happy to provide a copy of the orders to have them labs completed elsewhere.
Controlled Substances
Our office is not accepting patients that require controlled substances management. We are happy to provide preventive care or care of other chronic issues but will not be providing controlled substances. We can provide a referral to a pain management specialist, psychiatrist, or other specialists if you are not therapeutic with other treatments rendered.
Divorce Decrees
This office is NOT a part of your divorces decree. Adult patients are responsible for their bill at the time of service. The responsibilities for minors resets with the accompanying adult.
